much clearer about your profile choice. You could develop further the case for ACP
in people with chronic conditions. The need for being explicit for the reasons for
ACP is paramount. This is more than about planning for when the persons loses
competence to make a decision. A clearly defined link between ACP and its use in
chronic disease management is required and then you can use SLE as an example.
Remember this is about AC and not an essay on SLE.

I would suggest that you talk about these issues and devise a plan that could be

A relevant condition is identified for discussion in terms of an anticipatory care
approach. It was not clear what the practice context was or the area profiled. This
could be included to show a specific area/ward/clinic where the Anticipatory Care
Plan could be utilised. Bringing in relevant national policy drivers would add to
the discussion and strengthen the argument for the development of the plan. Looking
at a community of people with the same condition would give you the opportunity to
discuss the need to include patients and their carers/families to have a voice in
the plan. The need for evaluation needs to be discussed and analysed as part of how
this plan is taken forward. There are sections within the assignment that allow the
reader to see the relevance and value of the proposed plan. Discussing health
promotion activities linked to a specific theory would help focus the discussion.

In detail, this redevelopment project entails a number of deliverables such as zoning six departments with respect to their use over time; acquisition of advanced technology medical equipments; development of 28 operating theater rooms; development of two multi-storey car parking lots; a number of far-reaching demolitions; and a suitable design for both the landscape orientation and the building design. The redevelopment project is anticipated to take five years after its commencement.
