the assignment. You have mentioned SLE and outpatient clinics but you need to be
much clearer about your profile choice. You could develop further the case for ACP
in people with chronic conditions. The need for being explicit for the reasons for
ACP is paramount. This is more than about planning for when the persons loses
competence to make a decision. A clearly defined link between ACP and its use in
chronic disease management is required and then you can use SLE as an example.
Remember this is about AC and not an essay on SLE.
How could you use AC to help you and your patients visiting the outpatients clinics?
How can you use ACP?
I would suggest that you talk about these issues and devise a plan that could be
inserted as an appendix. Include a rationale for the choices made in your plan
Remember you only have 2000 words but an appendix isnt included in the word count.
You need to move away from talking specifically about all the elements of SLE.
In the main body of your text discuss how you would evaluate the strategies in
assessing the impact gains
Strengths and limitations of assignment:
A relevant condition is identified for discussion in terms of an anticipatory care
approach. It was not clear what the practice context was or the area profiled. This
could be included to show a specific area/ward/clinic where the Anticipatory Care
Plan could be utilised. Bringing in relevant national policy drivers would add to
the discussion and strengthen the argument for the development of the plan. Looking
at a community of people with the same condition would give you the opportunity to
discuss the need to include patients and their carers/families to have a voice in
the plan. The need for evaluation needs to be discussed and analysed as part of how
this plan is taken forward. There are sections within the assignment that allow the
reader to see the relevance and value of the proposed plan. Discussing health
promotion activities linked to a specific theory would help focus the discussion.

as to how they could improve their learning environment and/or the mentorship therein. The
proposal will indicate further evidence of the achievement of the mentor domains and should
be mapped to indicate which competencies this relates to.
This proposal aspect is academically marked at Undergraduate Degree Level (6) or Postgraduate
level (7) and so should be written in an effective literary style with the support of relevant
theoretical concepts appropriate literature and research. A reference list is required and should be
included at the end of the portfolio. To make the most of your proposal you must read and use the
School of Health and Social Care academic writing guidance which will be available to you at the
commencement of the programme.
The proposal should be structured with an introduction (following your SWOT analysis) which
highlights your intentions to develop an aspect of your learning environment and which NMC
mentor Domain/s this relates to.
The main body of your proposal should critically analyse/evaluate your key points with an effective
literary style approriate to your selected level with clear definition of topic and a logical structure.
Relevance/ application to practice with the use of supporting literature/synthesis and evidence of

In order to achieve the new forecasted capacities of the New Southmead Integrated community hospital, the redevelopment project has been flagged as a large project under the North Bristol NHS Trust. In detail, this redevelopment project entails a number of deliverables such as zoning six departments with respect to their use over time; acquisition of advanced technology medical equipments; development of 28 operating theater rooms; development of two multi-storey car parking lots; a number of far-reaching demolitions; and a suitable design for both the landscape orientation and the building design. The redevelopment project is anticipated to take five years after its commencement.

Phase I included implementation of an EHR system consisting of order entry for all laboratory, radiology, and patient care orders. Additionally, clinical documentation was implemented, including admission assessments and all nursing flow sheets. 1. This is Description to my organization and my role and responsibilities. I’m working in Medical City which is consists of four hospitals and four center. Main Hospital, Women’s Specialized Hospital, Children’s Hospital, Rehabilitation Hospital and Medical Centers which include Heart Center, Neuroscience Center, Hematology &Oncology Center and Specialized Diabetes & Endocrine Center. In addition to the hospitals there are primary care clinics, Emergency Medicine Administration and Faculty of Medicine. All these hospitals and centers consist of a total of 1,100 beds.

Since I finished my training and fellowship I handle many leadership positions and responsibility now I’m the chairman for Emergency medicine department which consists of 70 beds with around 100,000 visit per year. We have total of 18 attending physicians and around 30 resident in training program.
